A Seekonk teen has died following a two vehicle crash on I-495 in Raynham.

Massachusetts State Police say that at approximately 6:56 p.m. Wednesday troopers responded to a crash on I-495 South near Exit 8.

A preliminary investigation has determined that a 2002 Jeep Wrangler driven by 19-year-old Hayley Sanford of Seekonk was traveling in the right lane before it entered the middle lane and collided with a 2007 Honda CRV.

State Police say both vehicles were forced into the dirt median and the Wrangler rolled over ejecting Sanford from the vehicle.

An ambulance took her to Raynham Park where she was transported by helicopter to Rhode Island Hospital serious injuries.

She died later that evening.

The driver of the Honda, identified only as a 76-year-old New Bedford woman, was transported to Morton Hospital in Taunton with minor injuries.

The crash is being investigated by Troop D of the Massachusetts State Police, with the assistance of the State Police Detective Unit for Bristol County.
