I'd love to take credit for this. However, this form comes from the folks at Common Core Forum.

Many parents and grandparents have asked me what's the best way to opt their child/children from the Partnership for Assessment of Readiness for College and Careers (PARCC) tests.  First of all, the term "opt-out" will always grant you a stern "your child cannot opt-out" from teachers, principals and administrators. Second of all, fear not, the U.S. Constitution has your back, in particular, the Fourteenth Amendment. Third, you should know, that many families have already said no. From the few, comes a form many parents can now utilize, successfully, so their child/children can go to school, on PARCC test days, but not participate in PARCC.

Read thoroughly to comprehend how the U.S. Constitution and courts protect you!

My thanks again to the www.CommonCoreForum.com.
