The goal of the PWTF is to reduce healthcare costs by supporting evidence-based community and clinical prevention strategies to decrease preventable risk factors/illness and disparities and improve the management of existing chronic disease. The program will train and mobilize a diverse, culturally- and linguistically-competent workforce of Community Health Workers (CHWs) who will serve as an important link between clinical and community partners.
Manages and coordinates partnership activities. Manages SHIFT Program budget to be aligned with resource needs and ensures that the New Bedford Health Department and MDPH budget and expenditure requirements are followed. Prepares assigned aspects of required grant reports. Assists in developing and writing additional of grant applications to support additional activities related to program. Works in a confidential capacity with the New Bedford Health Director and clinical/community team members.
Ensures compliance with all funder (MDPH) requirements, including reporting and maintaining communication with the MDPH Project Officer. Acts as liaison between Coordinating Partner, all clinical and community partners, and CHWs. Collects all clinical referrals on a periodic basis and disseminate them to appropriate CHWs in a timely manner. Ensures CHWs are successfully following up on referrals. Analyzes data to provide insight to program successes and complications. Understands and provides strategies to successfully reach underserved populations. Operates independently. Qualifications:
Machine shop vocational graduate or equivalent
Five (5) + years work experience in the programming and operation of CNC equipment, 2-Axis, lathes and machining
Knowledge of manual machine shop equipment
Proficient in FeatureCAM or similar program
Must own appropriate tools
Must be able to work overtime as necessary
Experience with AutoCAD and SolidWorks is a plus
